Applicants should apply on line and print the application with ratification by the department head before deadline and submit the form to the Office of Academic Affairs by 17:00 at May 22 (Friday) for classes scheduled on Monday to Thursday, and 13:00 at May 25 (Monday) for classes scheduled on Friday. No application will be accepted after the deadline.
